Pulmonary Program

Our Pulmonary Care Program is designed to meet the needs of patients with pulmonary conditions post-hospitalization and patients with chronic pulmonary conditions.

Our interdisciplinary team provides care, treatment and services to patients who have:

 • Asthma • Bronchitis • Chronic Obstructive Pulmonary Disease (COPD)

• Emphysema • Lung Cancer • Pneumonia

Care and services are also provided after procedures or surgery such as:

 • Lung biopsies • Thoracotomy • Tracheostomy

A Physician-Led Interdisciplinary Team Approach to Care

Our Physician Advisor specializing in pulmonary care consults with an interdisciplinary team of attending physicians, nurses, therapists, dietitian and social worker to lead the Pulmonary Care Program. Patients and families receive individualized care with a focus on meeting their healthcare and discharge goals. In addition to building strength and endurance, patients receive education and preparation on how to take medications and learn new self-care skills. A simulated apartment allows therapists to complete final rehabilitation services and assess readiness for discharge.
