Orthopedic Care

Our Orthopedic Care Program is designed to meet the needs of patients post-hospitalization. Our interdisciplinary team provides care, treatment and services to patients who have had:

 • Hip fracture • Hip replacement • Total knee replacement • Fractures as a result of injury • Open reduction internal fixation (ORIF) • Other orthopedic surgical procedures

A Physician-Led Interdisciplinary Team Approach to Care

Our Physician Advisor specializing in orthopedic care consults with an interdisciplinary team of attending physicians, nurses, therapists, dietitian and social worker to lead the Orthopedic Care Program. Additional Physician Advisors with specialties in rehabilitation and post-acute care are available to consult with the team, based on the specific needs of the patient.
