Neurological & Post-CVA Care

Our Neurological and Post-CVA Care Program is designed to meet the needs of patients with neurological conditions post-hospitalization and patients who have had a cerebrovascular accident (CVA) or stroke.

Our interdisciplinary team provides care, treatment and services to patients who have:

 • Seizures • Traumatic Brain Injury (TBI) • Cerebrovascular Accident (CVA) • Fractures as a result of injury • Other neurological changes as a result of illnesses or injury

A Physician-Led Interdisciplinary Team Approach to Care

Our Physician Advisor specializing in neurological care consults with an interdisciplinary team of attending physicians, nurses, therapists, dietitian and social worker to lead the Neurological & Post-CVA Care Program. Patients and families receive individualized care with a focus on meeting their healthcare and discharge goals.

The program focuses on areas such as:

 • Self care skills including eating, grooming, bathing and dressing

• Mobility skills including walking, transfers or wheelchair mobility • Using adaptive equipment • Communication skills • Cognitive skills including memory and problem-solving

• Social skills
